Registration for Memberships

Please Note:  You will NOT be charged or asked for payment to register. All prices seen are the standard retail prices — before any discounts.  Again, you will NOT be charged, given a bill, or asked for ANY payment information during registration.

What is Registration? You will be providing your basic contact information only. This will 1) put a timestamp on your registration and secure your discounts and 2) allow you to set an appointment to discuss to enrollment as a Long Life Med patient member. Only a few slots are available in each membership category (see below). Once they are filled, we will close patient member enrollment until the next round of membership openings estimated to be in 30 to 90 days.

We are Not a Health Insurance Services Organization.  We are a private provider clinic or Direct Primary Care (DPC) clinic.  We Do NOT accept health insurance for the membership fees or other services in-clinic, but you can use your insurance for imaging, labs and medications not covered by us.  

We apologize, but we are unable to accept Medicare/Medicaid patients to become Direct Primary Care Members as of this time, but please click the link below and Pre-Register anyway to join our WAITLIST, and we will notify you when we are able to accept Medicare.

However, please note that Medicare will still not cover the Membership fees, and will just cover the labs, imaging, medications, specialists, and other services not covered by your DPC membership.

We offer a limited number of Telehealth Membership plans, only available for those who are 16-30 years old and in reasonably good health, who do not need to be seen in the office regularly.
It is the most affordable membership, if you are unable to afford standard health insurance, but want to have “just-in-case” sick & urgent care visits, and with annual medical check-ups. This will only include up to 1 virtual visit per month, and in-office visits will still be charged although much at lower than typical urgent care fees (and no wait times, of course).
Please pick at least the Standard DPC for minimum and more inclusive membership plans, for more savings.

The form will allow you to estimate your fees depending on how many adult members there are in your family, the membership type and billing cycle you select. You will NOT be charged until after your first visit and you are confirmed as a Member. Applicable discounts will be added at the confirmation of your enrollment.
For those families interested in different types of memberships and add-ons, we can also discuss those at your first visit and apply the custom family & bundling discounts.
In order to give full attention and care to our patients, each membership plan will always have a limited number of patients, especially in the beginning, so if there are no slots available, please register anyway and we can put you on the waitlist when we are ready to open up more slots.
The Executive DPC, however, will always be capped at 30 members, as it requires a higher degree of commitment and service, including hospital advocacy.

Medical Weight Loss Management

The form will allow you to estimate your fees depending on the combination of plans you are interested in and billing cycle you select. You will NOT be charged until after your first visit and you are confirmed as a Member.
These are already highly discounted combos, all of them stacked (e.g. you get an additional 10% off the already discounted at 20% WL + TRT combo, if you pay annually and are also a member of the Standard DPC), and the highest tier DPC membership is practically getting the add-ons at cost (30% on top of the 20%).
Other applicable discounts (couple, family, first responder, military, medical, college student) will be added at the confirmation of your enrollment.
