Fatigue, brain fog, mood changes, low libido and weight gain can occur with hormone changes—but they can also come from sleep problems, thyroid disease, medications, stress, nutrient deficiencies and other conditions. Good hormone care starts by asking what is actually driving the symptoms.
Long Life Med provides individualized hormone evaluation and ongoing monitoring for adults in Las Vegas, Henderson, Silverado Ranch and surrounding Southern Nevada. NP David takes the time to review symptoms, medical history, medications, supplements, lifestyle and the right labs before discussing treatment.
What BHRT really means
“Bioidentical” means a hormone is chemically identical to one the body makes. Some FDA-approved estradiol and micronized progesterone products are bioidentical. Custom-compounded hormones can also be bioidentical, but compounded products are not FDA-approved and are not reviewed in the same way for safety, effectiveness or manufacturing quality.
Compounding can be appropriate for a specific need, such as an allergy or a dosage form that is not commercially available. It should not be presented as automatically safer, more natural or more effective than an FDA-approved option.
Who may benefit from an evaluation
Women may seek help for hot flashes, night sweats, sleep disruption, vaginal symptoms, mood changes or other perimenopause and menopause concerns. Men may seek evaluation for low-testosterone symptoms such as reduced libido, fatigue, loss of muscle or erectile changes.
Symptoms alone do not diagnose a hormone disorder. For suspected low testosterone, diagnosis generally requires symptoms plus appropriately timed laboratory testing, often including repeat morning measurements. Women need an individualized review of menopause stage, uterus status, bleeding history and personal risk factors.
The right labs are more than one number
Hormone evaluation includes regular testosterone testing (using the most accurate version, not the cheapest available), estradiol, progesterone, FHS & LH or PSA when clinically useful. Thyroid markers such as free T4 and free T3, cortisol and other labs are also added as needed and guided by the history. We also review medications, supplements, alcohol, nicotine, sleep and nutrition before blaming every symptom on hormones. Lifestyle, genes, and specific symptoms guide the treatment process, as each person is different and some people are also more sensitive than others.
Treatment is a monitored process
The route, dose and schedule depend on the diagnosis and goals. Options may include FDA-approved products or a compounded medication when there is a clear reason. Follow-up checks symptoms, side effects, blood pressure and appropriate labs, then adjusts the plan rather than leaving you on an unchanged prescription.
Hormone therapy should not be sold as a way to prevent heart disease or dementia. For many healthy women younger than 60 or within ten years of menopause onset, the benefit-risk balance can be favorable for bothersome symptoms, but the decision remains individualized.
How the FDA warning changed—and what did not
The FDA approved labeling changes for six menopausal hormone therapy products that removed certain class-wide statements about cardiovascular disease, breast cancer and probable dementia from the boxed warning. That update does not mean hormone therapy is risk-free or appropriate for everyone. Product, route, dose, age, timing and medical history still matter.
Care without the hormone-mill model
Long Life Med caps its roster at 300 patients. That structure supports extended visits, education and ongoing access with one primary provider instead of a one-time prescription. Hormone prescribing, monitoring and coaching can be coordinated with Direct Primary Care membership, with wholesale medication pricing available to members and non-members.

Frequently asked questions
Are all bioidentical hormones compounded?
No. FDA-approved estradiol and micronized progesterone products can be bioidentical. “Bioidentical” and “compounded” are not synonyms.
How quickly will symptoms change?
Timing varies by symptom, route and dose. Some people notice changes within weeks; others need adjustment or a different explanation for the symptoms. Each person is different, even twins, so treatments are individualized, depending on the person’s age, lifestyle, genes, symptoms, and other conditions.
Do you monitor treatment?
Yes. Ongoing symptom review, safety checks and appropriate labs are part of responsible hormone care. We don’t skimp on the required labs, and neither should you. There is a huge range on the acceptable lab values especially for women, but normal and optimal is different, and staying on the higher ends or over are not good for your health either, so we check labs and symptoms regularly. We are invested in your long term health, not just in short-term gains.
