This is for those prospective members who are interested in Executive DPC membership, Weight Management, Hormone Replacement Therapy for Men & Women, Anti-Aging, or any combination of these listed.
If you are doing more than one plan, please notify us by texting or emailing us, as we might need to adjust it for a much longer appointment.
The longer appointment time will allow us to do a full physical exam, lab draw, Q&A and finalize the enrollment as a member.

This longer appointment is for those prospective members who are coming in as a couple or a family so we can see you both at the same time.

This is an IN-CLINIC visit that requires the medical care PROVIDER to provide DIRECT CARE, which includes follow-up, blood draw, clinical education, and other clinical concerns.
If you are sick or injured, and need seen by your Provider sooner, please book either a TeleHealth visit or an Urgent Care visit. You can also text or call your Provider if you are not sure.
This is an IN-CLINIC visit that requires the medical care PROVIDER to provide DIRECT CARE, like physicals, longer follow-up, blood draw, clinical education, medical procedure, and a combination of other clinical concerns.
If you are sick or injured, and need seen by your Provider sooner, please book either a TeleHealth visit or an Urgent Care visit. You can also text or call your Provider if you are not sure.
Any sickness or injury that requires in-person testing and a physical exam should be booked here for an IN-CLINIC visit.

Please be detailed on what you plan to discuss or need to do with the Provider when you are booking this appointment, so we can be sure that we have enough time for the appointment, and so that the Provider can bring everything he will need for the house visit.
